2026 Online Parent-Administered Test Registration Form

FORM INSTRUCTIONS

Note: Any active special pricing will be applied at check out. If any special pricing is active, it will be announced on the Testing main page.

Provide:

  1. Parent’s name or guardian who will administer the test.
  2. The week the test will be given (i.e. 5/4 to 5/10).
  3. Parent name(s) and contact info.
  4. Mailing address to receive test results
  5. Provide first student name and grade level to be tested.
  6. Choose yes to order the optional Performance Evaluation Report. This report identifies specific objectives measured by all items on the test.
  7. Once information for the first student is entered, you may register up to two more students on this form.
  8. Complete “Test Selection and Order Total” section. Choose Confirm Registration and enter a quantity to confirm the total number of students you will be testing at grade 1-3 and/or for grades 4-12. Choosing Include in Order confirms the total number of Performance Evaluation Reports you are ordering.
  9. Choose your payment method now that your order has been totaled. You will be prompted to complete the payment process for this order.

If your payment and form submission was successful, you will receive an email from us at the email address that you provided on the form. This email will include a copy of the form for your records and reference.

ATTENTION Oregon Residents: The use and results of this test are for informational purposes only. Achievement tests must be administered by neutral and qualified examiners as defined by OAR 339.035 to satisfy state testing requirements. Choose one of our private, semi-private, or group testing options if you are an Oregon resident and are testing to satisfy testing the testing requirement.
