Remote Group Testing

Provide:

1. Click the circle next to the test date of your choice.
2. For “host”, put Basic Skills. If part of a private group, put the following in the “host” field: the group leader’s name, test location, and group test date.
3. Family information, parent names, mailing address (to receive test materials/test results), and email.
4. Student name(s) and grade level(s) to be tested.
5. Check yes to order the optional Performance Evaluation Report. This report identifies specific objectives measured by all items on the test.
6. Select the testing materials you are ordering to show the total cost. Testing fees will cover the following: Testing materials, basic report, and (if ordered) Performance Evaluation Report. Please note an additional processing fee of $8 is included in the price of testing for grades 1, 2, and 3.
7. Click Submit. Your registration form will be submitted to Basic Skills and you be directed to a payment page.

**If testing multiple students and one student is a 1st, 2nd, or 3rd grader, it will be necessary to go through the payment process twice: once for students grades 4-12, and again for students testing at 3rd grade.**
