Online Private Survey Test

FORM INSTRUCTIONS

Provide:

  1. Test date, test appointment time, and name of examiner.
  2. Family information, parent names, mailing address (to receive test materials/test results), and email.
  3. Provide first student name and grade level to be tested.
  4. Choose yes to order the optional Performance Evaluation Report. This report identifies specific objectives measured by all items on the test.
  5. Once information for the first student is entered, you may register another student for this test date by choosing yes to the question that follows. You may register up to 3 students for the same test date per form.
  6. Complete “Test Selection and Order Total” section. Choose Confirm Registration and enter a quantity to confirm the total number of students you will be testing at grade 1-3 and/or for grades 4-12. Choosing Include in Order confirms the total number of Performance Evaluation Reports you are ordering.
  7. Choose your payment method now that your order has been totaled. You will be prompted to complete the payment process for this order.

If your payment and form submission was successful, you will receive an email from us at the email address that you provided on the form. This email will include a copy of the form for your records and reference.
